This will allow you to download this software, providing access to the forms. Please bring the completed form/s with you to your scheduled visit. This will help expedite the registration process. Fees & PaymentsWe make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard and Visa for your convenience. If we are a participating provider of your insurance company, we'll bill them. However, payment is the patient's responsibility. We'll help in any way we can to assist you in handling claims.InsuranceWe're members of many Preferred Provider Organizations and are a Participating Provider for Medicare and accept most indemnity insurances. In addition we are approved providers for most Workers Compensation Carriers.Prescriptions & RenewalsPrescription requests should be made through your pharmacy. Your Pharmacist will contact our office on your behalf. We recommend that all prescription requests be received no later than 3:00pm for processing. Most prescriptions are filled the same day that they are received, however, we suggest that you allow 72 hours for processing of your refill request. Renewals requested at other times will be filled only for extreme or emergent circumstances. |
